After I have been running my truck and i turn it off, about 3 seconds later it backfires once, very loud. Curious, I turned it on then off again, it backfired only once again. Could this be the o2 sensor? It is also having trouble passing smog.

i would guess the timing. as thats really the only thing that will make a back fire. it could have jumped time. how long has it been since you changed the timing belt. also, does it ever diesel insted of back fireing?

one more question is it fuel injected or carburated. in a carbed engine a backfire can also be caused by a rich condition, but more than likely its the timing.
a leak in the exhaust may also cause this but it is less likely.

It is a carburated engine. I have never changed the timing belt. I just bought it a couple of weeks ago. It never diesels, just backfires once, after the engine is shut off.

I know it is off subject, and no real help to you, but I used to have this '74 VW Beetle that I could make backfire by applying a little backpressure. Thing is, it wouldn't just backfire and be loud, it would actually flame out of the exhaust. Let me tell you, when I had someone riding my ass at night, I'd just let off the gas, let the pressure build, and BOOM - light up the ass end of my bug and they would back off real quick.
Ahhh.... good times.

LOL, that is funny mike.
BOT, i would guess the timing needs adjusted. also how long has it been since it had a tune up? i would give her a good tune up, and check the timing. should fix her up.
