Here is my Hi-Lift slider adapter in the pictures and a link with all the specifics and ordering information below:
Hi-Lift Slider Adapter
and yours will be fabricated to the same dimensions. I have a 48" Hi-Lift and the adapter fits perfectly. Your particular jack, if not a Hi-Lift, might require some modification for a perfect fit and several folks have done so. This is a well built product and was fabricated with usability and simplicity in mind. I work in a fab shop as a fabrication engineer and laid out the basic design and my buddy Scott did most of the fabrication as he is a much better welder/machinist than myself. As you can see it more than handles the load of picking up the vehicle.
Item: (1) Hi-Lift Slider Adapter - bare carbon steel w/ 1/2" Grade 5 bolt/nut and rubber shield (loose). Due to possible chipping during shipment or the fact that you might not like red, I won't paint the adapter and I'll leave the rubber shield loose so you can glue it on after you paint the adapter the color of your choice. The adapter piping on top has a 2" ID so it will accommodate the beefiest of round tubing used on your sliders. The adapter slides over the existing lift plate for the Hi-Lift and uses slots on either side and the included bolt to attach to the lift plate. The lift plate will still take the bulk of the loading and the adapter is only to prevent the Hi-Lift from slipping off your round sliders due to the awkward shape and predominantly flat surface of the lift plate.
Cost: $35 shipped anywhere in the continental United States (this includes shipping and packaging).
